Red Bluff Physical Therapy Appointment

Please bring to your first Physical Therapy appointment:

  • Proper forms of identification

  • Insurance cards

  • Completed New Patient Form (download available below)

  • Completed Medical History Form (download available below)

  • Payment information

  • Doctor’s referral

  • Loose fitting clothing/shoes (This allows physical therapists to treat the injured area)

Each treatment can last anywhere from thirty to sixty minutes, depending on the techniques and treatments utilized. Communicate any pain encountered during or after treatment to the physical therapist so they can make proper adjustments and create a customized treatment plan designed specifically for your injury and to help reduce pain and provide relief.
